ADVERTISEMENT FOR BIDS Sealed BIDS for construction of the US 40 Sidewalk Improvements (Project # SHE M255-006, Project Code 22214) project will be received at the City Hall, Craig, CO until 10:00 AM June 19th, 2019 at which time they will be publicly opened and read aloud. Project is the construction of sidewalk improvements to the US 40 corridor including Crescent Drive and Victory Way and the associated transportation improvements. This will include removals, unclassified excavation, embankment, erosion control, asphalt roadway patching, curb and gutter, pedestrian sidewalk, drainage improvements, traffic control, and minor signing and striping. The project will also include Local Agency requirements from the Colorado Department of Transportation and the Colorado Department of Health and Environment. Bidders must be pre-qualified by the Colorado Department of Transportation. Only Bids from Pre-qualified bidders will be accepted. The CDOT prequalified contractor list is available at http://www.dot.state.co.us/Bidding/listings.html The CDOT EEO officer has set the Disadvantaged Business Goal at 2% for this project. There are no on-the-job training goals. The CDOT Form 347, Certification of EEO Compliance, is no longer required to be submitted in the bid package. This form certified that the contractor/proposed subcontractors were in compliance with the Joint Reporting Committee EEO-1 form requirements. The EEO-1 Report must still be submitted to the Joint Reporting Committee if the contractors and subcontractors meet the eligibility requirements (29CFR 1602.7); we will, however, no longer require certification. Bidder must include CDOT forms 606, 1413, 1414 with his bid. On the day of the bid opening, the low bidder will be given the following forms: Form 605, Contractors Performance Capability Statement Form 621, Assignment of Antitrust Claims Form 1415, UDBE Commitment Confirmation (If the UDBE goal > 0) Form 1416, UDBE Good Faith Effort Documentation (If the UDBE goal is not met) These forms need to be returned to the Local Agency by 4:30 pm the following day. A Bid Bond in the amount of 10% of bid price is required, and Performance and Payment Bonds in the amount of 100% of bid, will be required. Convene at City Hall, Craig, CO on June 11th, 2019 at 3:00 PM, for a mandatory pre-bid meeting.
